工学院では、受験者本人からの請求に基づき、令和６（2024）年度４月入学（令和５年８月22日、23日実施）入試に係る試験成績（修士課程）について、次の要領により開示しますので、希望者は請求期間内にお申し込みください。

◇開示方法

郵送による請求に基づき、書面で回答します。
なお、返送までに２～３週間ほどお時間をいただいておりますので予めご了承ください。
＊必ず本人が郵送で請求してください。窓口に直接持参しても受理できません。

◇開示内容

• 合格者の最高点／最低点
• 開示請求者の各科目の点数／総合点

受験者である本人
なお、1科目でも受験しなかった受験者の請求については、合格者判定の対象外であるため、試験成績を開示できません。

令和５年10月２日（月）から10月13日（金）まで（消印有効）

◇請求方法

書面（開示請求書）により、受験票（原本）、返信用封筒（長形3号：12cm×23.5cm、本人の郵便番号・住所・氏名を明記し、404円分の切手（簡易書留料を含む）を貼付したもの）を同封して、工学系事務部教務課大学院担当へ郵送により請求してください。
＊請求封筒の表（左下）には、「成績請求」と朱書きしてください。
＊受験票（原本）は確認後、返送いたします。
